Application

All referrals welcome. Individuals referred are contacted within a week by the intake worker to schedule an intake and to come in to sign consent forms for release of information. If there is a waiting list, individuals are contacted on a weekly to bi-weekly basis for a follow up and encouraged to utilize crisis intervention if in need.

Description of Services:

Supports and services available through Case Management include:

  • ongoing assessment and goal development;
  • crisis intervention and outreach;
  • supportive counseling and assistance with problem solving;
  • assistance in accessing treatment and support in addressing symptoms and following through on treatment recommendations;
  • coordination of services with natural supports and other agencies;
  • assistance in meeting basic needs; and
  • advocating with and on behalf of clients as necessary.
