Description of Services:

Mental health and substance use counseling as well as chronic disease monitoring, weight management and nutrition education

Services include:
  • 8-10 brief one to one and/group counselling services
  • co-ordinated care management
  • goal-focused plans based on the recovery and harm reduction models
  • extensive internal and external community referrals
