six-month program designed for individuals with known cardiovascular disease, focusing on therapeutic exercise and medical management. The program aims to optimize recovery after a cardiac event or procedure, reduce the risk of disease progression, and enhance quality of life.
Services include medical evaluation, guideline optimization, care coordination, supervised therapeutic exercise, self-management education, smoking cessation support, and nutrition and weight management assistance. Additionally, the Heart Failure Clinic is an outpatient program that operates on a referral basis, where a nurse practitioner collaborates with physicians and other nurse practitioners to optimize heart failure management.
