Description of Services:

Provides support services for recipients of Passport funds which can be used for but not limited to the following services:
  • Management of all the paper work in applying for Passport funds
  • Administration of the funds for desired supports and services; can cover transportation costs such as taxis/buses
  • Person centred planning
  • Community participation such as bowling, swimming, concerts, sports events etc, as directed by client
  • Out of town trips for shopping, concerts or visiting family/friends
  • Support in activities within client's own home such as cooking and crafts
  • Caregiver respite for family and caregivers
