Health care services available to accommodate at home, at school, and in the community, as well as access to supported living programs and long-term care * care coordinators work with people in the community or leaving the hospital to determine what help is needed and arrange for care

We are now Ontario Health atHome, a single organization coordinating local home and community care, long-term care placement and help finding services in the community. Our name has changed but our services remain the same. Patients continue to work with the same patient care teams and can contact us the same way.
