Description of Services:
Care for those affected by or living with the effects of a brain injury * partnership between Mackenzie Health's Centre for Behaviour Health Sciences and March of Dimes Canada
Services include:
case coordination and comprehensive functional assessment to evaluate the effects of the acquired brain injury on the individual and those who support them
individualized service/treatment plan, developed with recommendations and community support to promote the acquisitions of skills and strategies
plan which will enable persons with an acquired brain injury to live with maximal fulfillment in a state of dignity within the community
