Supports adults with developmental disabilities who are living independently or planning to live independently within their chosen community * designed to support through leading a life reflective of personal values and choices and assisting them in achieving individualized goals such as seeking accommodation, budget counselling, short-term emotional support, or respite care, among others

Program includes:

individualized planning based on the goals and support needs of the adult
ongoing case management
assistance in accessing and coordinating all necessary and available community services
