Outreach to homebound older adults in North Innisfil * primary health care

Services available:
  • clinic-based services
  • palliative care
  • care for housebound older adults
  • follow up with local nursing and retirement homes if admitted
  • cognition consultations

Consultations are in partner with the Barrie clinical team and the Aging Well Clinic of the Barrie health team
