Requires a signed consent form and completion of program intake form

Transportation Program: Must call ahead to make arrangements

Eligibility - Population(s) Served

Individuals 50 years of age and older

Individuals who are disabled, chronically ill and have special needs
Open to all with a focus on off-reserve Indigenous peoples

Transportation Program: Priority given for medical appointments

Description of Services:

Provides culturally appropriate and holistic long term support services and programs

Offers a weekly visiting session at Rainycrest Long-Term Care

Adult Day Program

Provides afternoons for socialization
Teaches various craft making sessions with focus on traditional Native crafts
Offers movie videos and card games

Congregate Dining

Provides a weekly luncheon consisting of full course meal, dessert and coffee
Offers craft making and time of socialization
Provides transportation for attendees

Friendly Visiting

Arranges weekly visits from support worker in individual's home
Provides emotional and physical support such as light housekeeping as required

Transportation Program

Provides transportation on an as needed basis to all events at the Centre, the hospital or clinic and for errands around town including grocery shopping
Arranges transportation for out of town specialist appointments when no other arrangements are available
