Eligibility - Population(s) Served

ALC or potential for Alternate Level of Care (ALC) in acute care who requires complex care needs upon discharge in the community; preferably those with active Long Term Care (LTC) placement application or waiting for crisis placement
Generally frail with multiple comorbidities
Patient needing behavioral support; must have a geriatric assessment

Description of Services:

Care for patients discharged from hospital/RCC that are deemed Alternative Level of Care (ALC) patients

Criteria for Program:
  • 55 years and above, recently discharged from the hospital deemed ALC
  • LTC application completed prior to admission to CANES Transition Care beds (length of stay is 42 days)
  • Ontario Health atHome-Brampton Placement Care Coordinator is informed about the admission and activate crisis status on discharge from hospital/RCC
  • For clients with the community as the post discharge plan, client and family discussion is in place before the admission from the program

  • Bramalea Retirement Residence (30 Peel Centre Dr, Brampton, ON L6T 4G3)
  • Southbrook Retirement Community (400 Ray Lawson Blvd, Brampton, ON L6Y 4G4)
  • Sorrento Retirement Residence (10 Station Rd, Bolton, ON L7E 4L3)
