Description of Services:

Community-based case management for individuals with serious mental illness and addictions who are in conflict with the law for minor offences.

Program includes:

  • ongoing assessments
  • treatment planning
  • coordination of required services
  • individual, group and family counselling
  • conflict resolution and crisis prevention
  • housing supports
  • discharge planning including referral to community resources
  • court diversion order coordination
