Provides support, resources, and advocacy for individuals and their support networks in urgent situations to prevent the further escalation of risk, mitigate any untenable risks, and address an unmet urgent need * Urgent Response Case Manager and Crisis Response Network Coordinator work collaboratively with community partner agencies as part of a multi-disciplinary team strategy to provide a temporary, short-term case management response.
