Brief Case management services are offered to individuals living with severe and persistent mental illness. Outreach services encourage independent living with a supportive counselling approach. Individuals of the service are offered a wide range of services including advocacy, monitoring of symptoms, education, empowerment, life skills, community re-integrations, housing support referrals and support in accessing other community services. We offer educational group sessions on various topics. Treatment plans are individually tailored to support a community-based recovery model.
