Although initial inquiry may come from patient/family member, a referral may be required from a hospital professional such as the physician, nurse, or social worker.
Waiting Period - Case management services are available prior to hospital discharge.
Eligibility - Population(s) Served
Must be a resident of Ontario with a valid Ontario Health Insurance (OHIP) card. Must be a patient in the Campbellford Memorial Hospital. Further eligibility will be assessed upon initial intake according to the Long Term Care Act (1994) (see attached).

Description of Services:
Provides in-home, information and referral, and placement services by care coordinators in Campbellford Memorial Hospital.
The role of the hospital care coordinator is to assess and arrange for required Ontario Health atHome in-home services, and/or placement services, upon discharge from the hospital and to link patients to other useful community resources.
In-home services may include personal care, nursing, physiotherapy, occupational therapy, speech language pathology, nutrition and social work.
