Waiting Period - No waiting time to make a new referral. Waiting periods for direct service vary depending on type of service required and priority need.

Eligibility - Population(s) Served

Visiting services are available to residents of Ontario of any age who have a valid Ontario Health Insurance (OHIP) card. Further eligibility criteria will be assessed by a case manager during the intake process based on the full eligibility criteria

Description of Services:

Care coordinators are responsible for assessing eligibility, determining service needs, developing service plans, authorizing provision of services, coordinating the delivery of multiple services and monitoring ongoing service needs for in-home and school support services * They also assist the client in planning for discharge when services are no longer required * Care coordinators are responsible for assisting ineligible clients to explore other options or services and making appropriate referrals to other services * The services provided through Ontario Health atHome may include:
  • personal support
  • nursing, physiotherapy
  •  occupational therapy
  • speech-language therapy
  • social work
  • dietetic services
  • mental health services
  • case management/coordination
  • medical supplies and dressings
  • hospital sickroom equipment
  • laboratory and diagnostic services
  • determination for drug coverage under the Ontario Drug Benefit (ODB) Plan
  • Determines eligibility for Adult Day Programs
