Description of Services:

Helps seniors and their caregivers navigate the health care system ensuring they are connected to the care and services that are needed and wanted * support when seniors are faced with life changes and significant care needs, including:

referrals to community resources and programs
client advocacy
crisis intervention
help with long-term care options

Education and information sessions include:

wellness sessions
counselling and tours
