Seniors aged 55 years or older, who need assistance home from either William Osler (Etobicoke General or Brampton Civic) or Headwaters Hospital

Description of Services:

Assists seniors to settle at home when discharged from William Osler Health System - Etobicoke and Brampton sites, and Headwaters Health Care Centre

How this service works:
  • trained Personal Support Workers (PSW's) meet the patient in their hospital room at planned discharge time, or if the patient is in the emergency department, they are met there
  • once discharged from the hospital, a Home at Last PSW will accompany the patient to a waiting vehicle
  • the senior is accompanied to their local community pharmacy, while assisting with any needed supplies before the trip home
  • on arrival, the PSW will assist the client to settle in their home safely
  • PSW can also stay with the client until family arrives or until the client feels able to manage on their own
