- Individuals 18 years of age and older who have been diagnosed with a developmental disability
- Documentation is required to support diagnosis of a developmental disability

Description of Services:
Specialized Service Coordination (SSC) program
- Uses a person-directed approach to help individuals identify their goals and the steps to take to reach their goals
- Assists people with direct funding to learn how to create their own support networks within their available funding
- Supports individuals who are living on their own to live as independently, safely, and securely as possible in the community
- Assists in developing strategies for coping and adapting to life in the community
- Ensures that individuals are informed of their rights and are treated with respect, fairness and dignity (Advocacy)
- Assists individuals in managing income support from Ontario Disability Support Program (ODSP) (Temporary Trusteeship)
- Helps individuals use natural supports as a way of enhancing lives and understand the range of opportunities that exist in the community
