Description of Services:
Educates patients about their medications
- Utilizes the Ontario Telemedicine Network (OTN)
- Works by Nurse and patient reviewing all medications with Pharmacist on the OTN who provides medication education, side effects and potential interactions
- Provides a medication schedule to each patient
- Records the findings in patient's Electronic Medical Records (EMR) for physician's awareness
- Reviews all medication lists for patients discharged from Atikokan General Hospital (AGH)
- Records all changes in patient's EMR
- Refers patients with numerous changes to a 1-hour OTN session with Pharmacist
- Medication reviews for patients referred to the Falls Prevention program
- Monitoring of the INR test results; makes phone calls to patients with their new orders and informs them of when it's time for the next blood test
