Primary health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurse practitioner and nurses * access to nurse, nurse practitioner, dietitian, social worker, occupational therapist, addictions counselor, pharmacist and psychologist through the family health team
