Description of Services:
Provides services that promote physical and mental health for community members
Collaborates with community partners to provide access to health care services and support
Provides health awareness and counselling, education and prevention workshops
Coordinates clinics/workshops in areas that may include Immunization, Sexual Health, Smoking Cessation and Communicable Diseases
Addresses individual health care needs by providing:
Intake services to determine health care needs of individual
Referrals to appropriate health care providers based on information in the intake assessment
Advocacy and support on behalf of individual with other health professionals
Home visits to the sick, elderly, new mothers and persons with health care needs
Assistance with coordinating local and long distance travel arrangements for medical appointments
Primary health care through Kashechewan Nursing Station, Weeneebayko Area Health Authority
